How do I order replacement goggles?

1. Check if your replacement goggles are covered under warranty
You may be eligible for free replacement goggles under our warranty. The best way to know if you are eligible is to check the warranty pamphlet that came with your goggles or the information on our website.
The FORM Smart Swim Goggles come with a two-year manufacturing warranty:

2. If your goggles are not covered under warranty
If your FORM Smart Swim Goggles are physically damaged due to user error, past the warranty period, or have been lost, active FORM members can purchase a new pair of goggles for 50% off. A 3-month minimum membership commitment is required when a replacement pair of goggles is purchased. Goggles purchased at 50% off do not include any free membership period.
